Community Living Connections: The Right Kind of Help, When and Where You Need It

Are you facing aging or disability issues? Community Living Connections can connect you with the right kind of help, when and where you need it.

Older adults, adults with disabilities, caregivers, family members, and professionals can call 206-962-8467 or toll-free 1-844-348-5464 (KING) and be connected to a caring, highly trained advocate who will give you easy access to information, individual consultation, and options for community resources. Staff can also determine if you or your loved one are eligible for certain programs and public benefits. Seattle Human Services’ Aging and Disability Services division funds these services, so they are free of charge to callers.

Are you an older adult needing transportation? Are you helping an older adult find suitable housing? Are you concerned about the long-term care needs of a loved one? Community Living Connections staff members are called “advocates” because they will do whatever they can to get you the help you need. Advocates have specialized training and extensive knowledge about community resources that allows them to answer questions and provide options to help you or your loved one live with dignity and enjoy the best quality of life possible.

No matter your concern, your call, consultation, information, referrals and many services are free of charge. (Respite care is available on a sliding-fee scale.)
